During a recent shift in Illinois, Dr. Hemlata Meena, an OB hospitalist was urgently called to assist in a high-risk delivery. The baby was in distress, showing poor heart tones, and immediate intervention was needed. Upon arrival, the hospitalist used a vacuum to aid in delivery, successfully bringing out the baby’s head. However, the team then encountered shoulder dystocia—a serious complication where the baby’s shoulders fail to follow.
With the support of the delivery team and a cooperative mother, the hospitalist performed standard maneuvers, ultimately delivering the baby using the posterior shoulder technique. The NICU team immediately assessed the newborn, who fortunately showed no signs of nerve injury or fractures. Although the mother experienced some additional bleeding, it was controlled without the need for surgery.
