Dr. Preetpal Grewal was on a busy shift when a patient came into the obstetric emergency department (OBED) with complaints of uterine contractions and leaking fluid (from premature rupture of membranes).
The patient was evaluated in OB triage and determined to be in early labor with preeclampsia symptoms including severely elevated blood pressure and a concerning urine protein creatinine ratio.
The patient also complained of decreased fetal movement. On the fetal monitor, she was noted to have decelerations, indicating the baby was in distress. After a consultation with the maternal fetal medicine specialist (MFM), the patient was emergently taken to the operating room.
